About the Role

The Licensed Vocational/Practical Nurse (LVN/LPN) provides high-quality, patient-centered care in home settings, supporting patients on their health journey with a focus on preventing hospitalizations and delivering compassionate support to patients and families.

Responsibilities

• Verify and implement the Plan of Care
• Document skilled nursing notes
• Communicate significant changes to the healthcare team
• Ensure patient safety and promote self-care
• Suggest care plan revisions as needed
• Administer medications
• Collaborate with RNs and physicians

Requirements

• Graduate of an accredited nursing program
• Current LVN/LPN license
• At least 1 year of clinical experience (home health or hospice preferred)
• Ability to travel and lift up to 50 pounds
• Reliable transportation
• Excellent communication and teamwork skills
