Licensed Vocational / Practical Nurse

We are seeking a Licensed Vocational / Practical Nurse who is committed, compassionate, and ready to help us remain an industry pioneer in delivering unparalleled care.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Teach patients and caregivers self‑care techniques to support health.
• Collaborate with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency‑room visits through proactive patient care.
• Deliver compassionate, high‑quality care that supports positive patient outcomes.

Requirements

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited certificate, diploma, or associate school of nursing.
• At least 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 lb, including positioning or transferring patients and moving equipment.
• Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home‑health environment is preferred.
