Licensed Vocational Nurse - PDN

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Deliver skilled nursing services according to prescribed orders.
• Monitor signs of physiological and psychosocial changes in patients.
• Collect, analyze, and document data to identify nursing diagnoses.
• Provide quality, patient-centered nursing care in patients' homes, ensuring safety and adherence to medical orders.
• Assess patient conditions and communicate findings effectively.
• Maintain accurate documentation and follow all regulatory and safety standards.

Qualifications & Skills

• Current LVN license in the practicing state.
• Valid TB or Chest X-Ray and BLS certification.
• Preferably one year of professional nursing experience.
