Licensed Vocational Nurse (LVN) - Per Diem - Assisted Living

Rate of pay: $36.00 - $39.00 per hour with additional differentials of $1.25 per hour for the PM shift and $1.50 per hour for the NOC shift.

We are seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to provide basic therapeutic, restorative, and preventive care to residents while supporting their independence and well-being. You will collaborate with residents, families, physicians, and caregivers to implement individualized care plans and ensure the highest quality of care. The LVN also provides direction and supervision to care staff in accordance with community policies and state regulations.

Responsibilities

• Advocate patience, cheerfulness, and a positive work environment.
• Remain patient, level-headed, and cool under pressure.
• Provide initial emergency medical response to residents needing assistance.
• Initiate and coordinate requests for emergency medical care, including contacting physicians, ordering emergency (911) or non-emergency ambulance transfers, and conducting initial assessments of resident medical needs.
• Administer medications and treatments, including dressing changes, as ordered.
• Provide direct nursing care to residents within the guidelines of RCFE regulations and facility policy.
• Document nursing care in support of planned outcomes.
• Consult with the Director of AL/SCR regarding desired patient outcomes and progress or lack of progress.
• Identify changes in resident status that interfere with the ability to meet basic needs and report problems to the Director of AL/SCR for modification of the plan of care.
• Observe and assess effects of medications, report findings to Director of AL/SCR, and document appropriately.
• Attend AL Department meetings as scheduled and complete shift reports.
• Complete all online training scheduled by the Director of Staff Development and Director of AL/SCR.
• Assist in the effective operation of Casa de las Campanas and participate in relevant facility activities.
• Evaluate AL CNAs under the direction of the Director.
• Order and maintain medications.
• Keep accurate maintenance of medical records.
• Assist with prepping paperwork for any discharge of residents from Health Center back to AL/SCR.

Requirements

• Current licensure by the California Board of Vocational Nurse and Psychiatric Examiners.
• One year of experience as a Licensed Vocational Nurse in assisted living, long-term care, or a similar setting.
• Strong communication, clinical, and leadership skills.
• Compassionate and patient-centered approach to care.
• Ability to work collaboratively in a fast-paced environment.
• Must pass background checks, including a fitness-for-duty physical, fingerprinting, and employment references as required by a licensed residential care facility.
