Licensed Vocational Nurse (LVN/LPN)

About the role

This full-time position expects 30 productivity points completed per week. The clinician is paid per point and has the opportunity to earn additional pay for exceeding 30 points weekly. You will serve approximately 6 patients a day, Monday through Friday.

Responsibilities

• Provide comprehensive care in the patient's home, including skilled documentation through Homecare Homebase.
• Serve as the patient's advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
• Administer medications and treatments as prescribed by a physician.
• Provide direct patient care including infusion therapy, wound care, teaching, and disease management.
• Participate in regular case conferences and in-services.
• Perform any skilled procedure within the scope of the Nurse Practice Act as ordered by a physician.

Requirements

• Current Texas LPN license.
• Three years of clinical nursing experience.
• Current Texas driver's license.
• CPR (BLS) certification.
• Experience with healthcare software and EMR (Homecare Homebase).
• Comfortable treating wounds and performing infusion therapy.
• Strong organization skills, time management, and ability to multitask.
• Professional appearance and effective communication skills.

Pay

$39.10 per hour (paid per productivity point).

Schedule

• Monday to Friday.
• On the road (home health visits).
