Licensed Vocational Nurse (LVN) - ECM
About the Role
The Licensed Vocational Nurse (LVN) is a vital member of the Enhanced Care Management (ECM) team at Pacific Health Group, supporting high-risk Medi-Cal members with complex medical, behavioral health, and social needs in Hiring County. Working under the supervision of a Registered Nurse Clinical Case Consultant, the LVN assists with care coordination, member assessments, health education, medication reconciliation, transitions of care, chronic disease management, outreach, and clinical documentation.
Responsibilities
- Clinical Care Coordination: Support care coordination for CalAIM ECM program members, assist with treatment plans, conduct clinical outreach, monitor member progress, and identify barriers to care.
- Member Assessments & Clinical Support: Conduct health screenings, collect health information, assist with chronic disease management, monitor symptoms, and provide health education.
- Medication Management Support: Perform medication reconciliation, educate members on medications, monitor adherence, and coordinate medication-related resources.
- Community Outreach & Member Engagement: Conduct community-based outreach, participate in events, collaborate with partners, and promote Pacific Health Group programs.
- Field-Based Member Support: Conduct in-home and community visits, support member engagement, and ensure safety during visits.
- Collaboration & Interdisciplinary Team Support: Work with interdisciplinary teams, participate in meetings, and support integrated care initiatives.
- Documentation & Compliance: Maintain accurate documentation, ensure compliance with regulations, and protect member confidentiality.
- Quality Improvement & Professional Development: Participate in quality initiatives, attend training, and maintain licensure and certifications.
Qualifications
- Current and unrestricted California Licensed Vocational Nurse (LVN) license.
- Minimum one (1) year of clinical experience in healthcare, managed care, home health, public health, behavioral health, post-acute care, or community health settings.
- Experience working with underserved or vulnerable populations preferred.
- Strong understanding of care coordination and chronic disease management principles.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and collaboratively within interdisciplinary teams.
- Basic proficiency with electronic health records and Microsoft Office applications.
- Valid California Driver's License (Class C minimum) and reliable transportation.
Preferred Qualifications
- Experience with CalAIM ECM, Whole Person Care, Health Homes Program, or similar care management programs.
- Experience working with Medi-Cal populations.
- Experience supporting individuals with homelessness, serious mental illness, substance use disorders, justice involvement, or multiple chronic conditions.
- Bilingual in Spanish, Vietnamese, Tagalog, Punjabi, Hmong, or other languages commonly spoken in Hiring County communities.
- Community-based healthcare experience.
Requirements
- Valid California Driver's License and active auto insurance meeting CA requirements.
- Reliable personal vehicle for daily work use.
- Successful completion of background check (including MVR).
- Ability to travel up to 60-70% within the county for in-person visits.
- Successful completion of a Testlify skills assessment.
- Reliable working laptop for the first 21 days of employment (personal equipment stipend provided until company laptop is issued).
- Effective time management skills.
- Internet speed of 300+ Mbps download and 25+ Mbps upload.
- Proficiency in technology, including documentation systems and communication tools.
Work Environment & Travel Requirements
This is a hybrid position requiring a combination of remote work, office-based work, and field-based activities. Frequent travel throughout Hiring County is required for member visits, community outreach, provider meetings, and collaboration with partners. Occasional evenings and weekends may be needed.

Compensation: $30.00 - $35.00 per hour
