About the role
Hospice of South Texas is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to provide direct patient care in a home-based setting. Working under the supervision of a Registered Nurse, the LVN plays a critical role in supporting patients and families through end-of-life care while collaborating closely with the interdisciplinary team.
Key Responsibilities
- Deliver skilled nursing care in accordance with the plan of care (POC) under RN supervision
- Support the RN in implementing and coordinating patient care needs
- Auxiliary patients and families with education on self-care and disease management
- Document all clinical care, interventions, and patient responses accurately and timely
- Communicate changes in patient condition to the RN and care team
- Auxiliary with procedures and specialized care related to end-of-life support
- Prepare and maintain equipment and supplies using proper aseptic techniques
- Ensure care is delivered in compliance with organizational policies and regulatory standards
- Promote the mission, values, and quality standards of Hospice of South Texas
- Participate in interdisciplinary collaboration to ensure continuity of care
- Perform other duties as assigned
Qualifications
- Education & Licensure: Graduate of an accredited vocational nursing program, current Licensed Vocational Nurse (LVN) license in the State of Texas, valid Texas Driver's License
- Experience: Minimum of 2 years of clinical nursing experience, hospice or home health experience preferred
- Skills & Abilities: Strong clinical nursing skills aligned with accepted standards of practice, excellent communication and interpersonal skills, ability to respond effectively in emotionally sensitive situations, strong documentation and organizational skills
- Other Requirements: Reliable transportation and valid auto liability insurance, proof of Hepatitis B vaccination or declination
Work Environment & Physical Requirements
- Provide care in patients' homes with varying conditions
- Frequent standing, walking, bending, and lifting (up to 50 lbs.)
- Exposure to bloodborne pathogens and infectious diseases
- Ability to work flexible schedules, including on-call or PRN emergency situations
- Local travel required
