Licensed Vocational Nurse (LVN) - Adult Home Health

Role: Licensed Vocational Nurse (LVN) providing skilled adult home health care in patients' homes. Responsibilities: Conduct comprehensive assessments, administer medications and treatments, manage wounds, monitor chronic conditions, and document in the EHR. Collaborations: Work with RNs, therapists, and physicians to follow individualized care plans, educate patients and families, and ensure safety and independence at home. Qualifications & Skills: Nursing license with relevant home health experience, strong assessment, medication administration, wound management, and documentation skills, and ability to educate and support patients and families.
