Licensed Vocational Nurse, Hospice
About the Role
Provide one-to-one, compassionate care to patients in their homes as a Hospice LVN with Saint Agnes Home Health and Hospice. This role leverages cutting-edge technology and clinical expertise to deliver exceptional, patient-centered care. Our pioneering care model includes Home Care Connect, an integrated virtual care program that helps patients avoid preventable ER visits and hospitalizations through advanced remote monitoring and 24/7 access to our Virtual Care Center RNs.
As a Hospice LVN, you will be responsible for client assessment, care planning, intervention, and overall supervision of patient care and outcomes. You will follow physician orders and comply with the state's Nurse Practice Act, licensure requirements, and our policies and procedures.
Responsibilities
- Provide one-to-one care to patients in their homes.
- Conduct client assessments, develop care plans, and implement interventions.
- Supervise patient care and outcomes in compliance with physician orders and regulatory standards.
- Utilize advanced technology, including remote monitoring, to enhance patient care.
- Perform phlebotomy and wound care as required.
- Administer medications in accordance with licensure and certification requirements.
Requirements
- Graduate of an accredited or state-approved school of practical nursing.
- Licensure as an LVN in the State of California.
- A minimum of two (2) years of experience in an acute care setting; home care experience preferred.
- Phlebotomy and wound care experience required.
- Must have passed the National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

Pay
Pay range is $27.84–$44.55 per hour.
