Your Role: Home Health LVN
- Provide skilled home health nursing care based on individualized care plans.
- Deliver care to geriatric and senior patients in home-based and assisted living settings.
- Accurately document visits, treatments, and assessments in the EMR system.
- Collaborate with physicians, RNs, families, and interdisciplinary care teams.
- Support patients with chronic conditions, recovery, and dementia care needs.
- Ensure compliance with California home health regulations and company policies.
- Promote patient independence, safety, and overall well-being.
Qualifications
- Active California Licensed Vocational Nurse (LVN) license in good standing.
- Graduate of an accredited vocational nursing program.
- Minimum 1 year of nursing experience (Home Health experience preferred).
- Experience with geriatric patients, senior care, or dementia care preferred.
- Valid driver’s license, reliable transportation, and auto insurance.
- Strong communication, time management, and documentation skills.
- Comfortable using electronic medical records (EMR systems).
