About the Role

Men’s Health Foundation is seeking a compassionate, mission-driven Licensed Vocational Nurse (LVN) to manage individualized patient care by promoting and restoring patients' health through the nursing process. This role collaborates with physicians and multidisciplinary team members to provide physical and psychological support to patients, ensuring a compassionate and inclusive environment.

Responsibilities

  • Provides interdependent functions that overlap the practice of medicine, following standardized procedures and organizational policies.
  • Processes CMR reports to the county for reportable (+) STDs.
  • Assists with Prior Authorizations and reviews patient labs.
  • Triages patient phone calls, portal, and voicemail messages.
  • Administers medications, draws blood samples, and starts IV access (if certified).
  • Supervises workflow of Medical Assistants under the direction of the Nurse Manager.
  • Ensures clinic runs efficiently and on time.
  • Assists with patient outreach for PrEP and PEP patients, ensuring follow-up with the Prevention Coordination care team.
  • Operates within the scope of practice per the CA Nurse Practice Act.
  • Demonstrates in-depth knowledge of medical and clinical procedures and terminology.
  • Performs assessments and treatments per nursing care plan or as ordered by the medical provider.
  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Promotes patient independence by setting care goals and educating patients and families.
  • Adheres to therapeutic standards and measures health outcomes against patient care goals.
  • Resolves patient problems using multidisciplinary team strategies.
  • Maintains a safe and clean working environment by complying with procedures and regulations.
  • Protects patients and employees by adhering to infection-control policies and protocols.
  • Maintains nursing supplies inventory and ensures proper equipment usage.
  • Reviews lab values, performs medication reconciliation, and reports abnormal findings to providers.
  • Documents patient care services accurately in patient and department records.
  • Maintains patient confidentiality and protects operations by keeping information secure.
  • Provides care in a non-judgmental, non-discriminatory manner, respecting patient diversity and autonomy.
  • Attends and participates in staff meetings, case conferences, in-services, and other assigned meetings.
  • Monitors compliance with OSHA, HIPAA, and other applicable regulations.
  • Maintains professional knowledge through educational workshops, publications, and certifications.
  • Performs other duties as assigned.

Qualifications

  • Valid LVN license in the state of California.
  • Current Basic Life Support certification.
  • Proof of Flu and COVID-19 vaccination required on the first day of hire.
  • Minimum of two years’ experience in direct patient care as an LVN.
  • Experience in HIV/AIDS, Hepatitis C, and chronic disease care/management preferred.
  • Basic knowledge of infectious diseases and state/federal reporting requirements.
  • Demonstrated ability to address psychosocial needs, trauma history, and cognitive/behavioral challenges.
  • Behavioral maturity, patience, diplomacy, and tact in handling diverse patient needs.
  • Ability to work effectively with men and transgender women of diverse backgrounds in a multicultural environment.
  • Additional certifications considered an advantage.

Requirements

  • Must pass a pre-employment drug test, physical, and background check (7-year criminal, 10-year SSN & employer history).
  • Excellent interpersonal skills and attention to detail.
  • Ability to work flexible schedules.
  • Must take yearly flu shot or wear a flu mask during flu season for patient-facing positions.
  • Must test for tuberculosis as required by the Centers for Disease Control and Prevention.
