Licensed Vocational Nurse

About the Role

Assesses, plans, and implements the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse and Director of Nursing. Responsible for administering medications following the eight (8) medication rights to reduce the potential for medication errors. Ensures continuity of care between shifts by providing direct care and supervising CNAs and supportive staff. Participates in resident and family teaching while maintaining a role as resident advocate with a focus on the facility's mission.

Responsibilities

• Assesses the resident's condition and nursing needs, prescribes appropriate nursing action.
• Adequately assesses and reassesses pain, utilizing appropriate pain management techniques and educating residents and families regarding pain management.
• Documents changes in residents' condition and nursing care on progress notes and care plans.
• Performs waived testing (point-of-care testing) per Clinical Laboratory's and resident care unit's policies and procedures; interprets results and takes appropriate action.
• Administers medications following the eight (8) medication rights to reduce potential for medication errors and meets current documentation standards.
• Notifies physician of changes in resident's condition and follows through until appropriate action is taken.
• Notes and conducts physician's orders in a timely manner.
• Provides a thorough report to the oncoming shift by participating in walking rounds and documenting upcoming needs.
• Follows through on information given by shift report, resident, or family regarding resident concerns.
• Implements the resident's plan of care based on assessments and goals established by the interdisciplinary care team.
• Treats residents and their families with respect and dignity, addressing psychosocial, cultural, ethnic, and religious/spiritual needs.
• Supervises other members of the healthcare team to accomplish the plan of care.
• Demonstrates competence in technical and manual skills according to the scope of practice for LVNs.
• Functions as a resident advocate by protecting the resident's rights.
• Demonstrates creativity in solving problems and approaching situations in innovative ways.
• Interacts with residents, families, co-workers, managers, and other departmental personnel in a kind, knowledgeable, and friendly manner.
• Consistently involves residents and family in planning for care, treatment, teaching needs, and discharge plans.
• Functions as a leader for CNAs, delegating in a manner that contributes to the excellence of the nursing unit.
• Participates in the orientation of new personnel and assists in performance evaluations when needed.
• Follows proper infection control procedures and supervises supportive staff in these procedures.
• Demonstrates the ability to remain calm in emergencies and stressful situations, instilling confidence in the team.
• Consistently follows the policies and procedures of the facility.
• Exhibits genuine interest in and appreciation for adults in later years of life.
• Organizes workload independently to ensure all functions are completed appropriately.

Requirements

• Graduate of an accredited LVN program.
• Current California Licensed Vocational Nurse license.
• Current BLS certification.
• Ability to read and communicate effectively in English.
• Basic computer knowledge.
