LVN/LPN Position Details
Location: Thousand Oaks, CA
Department: Infusion Services
Schedule: Monday-Friday Shift Day Shift
Hours: 8:00 AM - 4:30 PM
Required Qualifications
- Licensure & Certifications: Active California LVN License, Current BLS Certification, California IV Certification for LVNs
- Experience: Minimum 1 year of LVN experience, Epic EMR experience required, Outpatient clinic experience preferred, Oncology experience preferred
Primary Responsibilities
- Administer injections and medications
- Access implanted ports
- Perform peripheral blood draws
- Start and maintain IV access
- Affirm simple infusions, including hydration therapy
- Monitor patients during treatment
- Obtain and document vital signs
- Communicate patient status changes to RN and providers
- Provide patient education and support
- Document care accurately in Epic
