Licensed Therapist (LCSW, LPC, LMFT)
About the role
Imagine empowering individuals within a dedicated patient community to overcome challenges like anxiety, depression, trauma, and addiction. As a trusted clinical partner, you'll deliver compassionate, whole-person, integrated mental health care, guiding patients through stressors related to relationships, family, parenting, and more.
Responsibilities
- Direct Counseling: Provide compassionate, whole-person care, offering individual and/or family counseling to help patients manage stress, improve relationships, and develop healthier coping strategies.
- Care Coordination: Seamlessly connect patients with long-term therapy, psychiatric services, or vital outside resources when needed, ensuring continuity of care.
- Integrated Collaboration: Collaborate closely with medical providers to gain a holistic understanding of each patient's health journey and contribute as an integral member of the integrated care team.
- Outreach & Education: Champion engagement by raising awareness of mental health services through client-driven wellness initiatives, proactive check-ins, workshops, and educational programs.
- Resource Navigation: Identify and address social determinants of health (like access to food or transportation), bridging the gap to necessary community resources to enhance overall well-being.
- Team Building: Cultivate strong, collaborative relationships with health center staff and leadership, adapting to evolving center and patient needs.
Requirements
- Education: A Master’s in Social Work (M.S.W.) from a CSWE-accredited program—or a related behavioral health field.
- Licensure: A current, active license with the Board of Social Work (or related licensing board) in good standing within the practice state.
- Treatment Experience: At least 12 months of recent (within the last 2 years) experience providing evidence-based individual counseling in a mental health or healthcare setting.
- Tech Skills: Comfortable navigating electronic medical records, clinical documentation, and basic word processing/data entry (ideally 12 months of recent experience).
- Preferred Experience: Previous experience providing integrated behavioral health services in a primary care setting within the last 3 years.
