Licensed Social Worker (Skilled Nursing/Long Term Care)

Park Manor of Conroe is seeking an experienced and dedicated Licensed Social Worker! Job Expectations: Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being. Requirements: Must have experience with PointClickCare 2 Years Social Work experience in a Long Term Care or SNF 4 year Degree Licensed Social Worker in the State of Texas Job Responsibilities: Resident/Family/Physician Intervention Psychosocial Services Vision, Hearing, Dental Services Assessments & Care Planning Discharge Planning Education / Qualifications: · 4 year Degree · Social Worker License Knowledge / Experience: · At least 2 years experience in a Skilled Nursing Facility or Long Trm Care REQUIRED Licensure / Certifications: · Social Worker License Benefits: Full-time We are equally committed to both our residents and employees and have created an environment that promotes professional and personal growth.
