Licensed Social Worker LSW
About the role
The Licensed Social Worker (LSW) is a member of the Quality Management team and collaborates with members of the interdisciplinary team (patient, family, provider, RN Case Managers, nurses, therapist, etc) in case finding/screening, assessment, planning, implementation, evaluation, monitoring and documentation of the patient’s care, psychosocial, and discharge needs.
Requirements
Education: LSW Social Worker and/or LSW Social Worker with MSW supervision if indicated by regulations with at least two years of experience, preferably inclusive of experience in an acute care or Clinical Social Worker required.
License/Certifications: Current license with the Maine State Board of Social Work required.
Experience: At least two years of experience, preferably inclusive of experience in an acute care or Clinical Social Worker.
Skills
- Certification or education in Case Management (preferred)
