Brief Description

Mountain Park Health Center is seeking Licensed Professional Counselors (LPC) to join their team. They are a well-established and accredited FQHC in Arizona dedicated to providing excellent, patient-centered primary care.

About the role

You will coordinate with primary medical and behavioral health teams to ensure comprehensive care to patients. You will work as an integrated team member in one of their primary care clinics.

Responsibilities

• Assesses patients and creates, coordinates treatment plans.
• Oversight of ongoing treatment of assigned patients.
• Provides brief, evidence-based, solution-focused behavioral interventions.
• Provides quality clinical consultation to patients and providers regarding mental, behavioral, emotional, and physical health issues.
• Screens and assesses behavioral health issues for patients.
• Collaborates with medical providers, support staff, and operational managers to identify patients needing behavioral health interventions.
• Develops and implements clinical pathways for select populations.
• Facilitates groups addressing biopsychosocial issues impacting medical issues.
• Coordinates care and access to behavioral health agencies in the community.
• Facilitates crisis triage and intervention.
• Helps de-escalate patient situations.
• Maintain regular and predictable attendance.

Qualifications

• Current licensure in the state of Arizona as a Professional Counselor.
• Eligible for credentialing and contracting with all insurers.
• Valid CPR card.
• Training in Motivational Interviewing (MI).
• Training in Acceptance and Commitment Therapy (ACT).
