About the role

Great opportunity for a therapist/counselor who is passionate about patients with neurological injury—such as traumatic brain injury, stroke, and spinal cord injury—to provide counseling and functional therapy supporting recovery in our unique post-acute neuro setting. Neurological injuries often lead to sudden, life-altering changes in function, independence, and self-identity.

This position involves providing one-on-one counseling sessions, facilitating social adjustment groups, and participating in functional patient activities and outings as part of a trans-disciplinary team including PT, OT, SLP, and Nursing at the Neuro Transitional Center, a 12-bed facility.

Responsibilities

  • Conduct comprehensive initial assessments within scope of practice
  • Collaborate with the transdisciplinary team to determine patient expected outcomes
  • Conduct individual and group counseling sessions within scope of practice
  • Promote patient and family understanding
  • Participate in patient team conferences and family conferences
  • Provide cognitive rehabilitation therapy addressing identified impairments, incorporating personally relevant functional activities based on patient interests and goals
  • Participate in goal-directed community integration activities
  • Incorporate formal neuropsychological test results into the plan of care, therapy tasks, and behavioral management plans
  • Develop behavioral plans for patients, educate workforce members to ensure consistency, monitor, and adjust as needed

Requirements

  • Master’s degree appropriate to the position
  • Licensed Professional Counselor (LPC)
  • Certified BLS completion required (for new hires, see CPR Requirement Guidelines for New Hires)

  • Demonstrated accuracy, reliability, and organizational skill

Schedule

Part-time, 20 hours per week
