Responsibilities
- Conduct behavioral health assessments and develop individualized care plans
- Gather patient histories through interviews with patients and families
- Provide individual, couples, family, group, and crisis counseling services
- Support wellness management and coordinate referrals for specialized care when needed
- Collaborate with providers, psychiatrists, and interdisciplinary care teams
- Educate patients on behavioral health conditions and health-promoting activities
- Ensure timely and accurate documentation of all patient interactions
- Manage follow-up care and ensure timely communication with patients
- Support integrated care coordination across behavioral health, primary care, and community resources
- Perform additional duties as assigned within scope
Requirements
- Active and unrestricted Texas license as an LCSW or LPC
- Ability to practice independently
- Eligibility for Medicare/Medicaid provider participation
- At least one year of clinical experience in area of practice
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Ability to handle sensitive situations and complex patient needs
- Strong collaboration and care coordination skills
- Commitment to patient-centered care and positive patient outcomes
Preferred Qualifications
- Bilingual in Spanish
- Knowledge of public health principles and integrated care models
- Experience working with substance use disorders (SUD)
- Five years of clinical experience
