Brief Description

Mountain Park Health Center is seeking Licensed Professional Counselors (LPC) to join their team. They are a well-established and accredited FQHC in Arizona committed to providing excellent, patient-centered primary care.

About the Role

Licensed Professional Counselors work as part of an integrated team in one of the clinic's primary care clinics. They assess patients, create and coordinate treatment plans, and oversee the ongoing treatment of assigned patients. Additionally, they provide traditional counseling/therapy to pediatric and adolescent populations.

Essential Functions

  • Provides brief, evidence-based, solution-focused behavioral interventions with active development of psychotherapeutic/psychoeducational behavioral change planning for patients.
  • Offers quality clinical consultation to patients and providers on mental, behavioral, emotional, and physical health issues.
  • Conducts screening and functional assessments of behavioral health issues for patients.
  • Collaborates with medical providers, support staff, and operational managers to identify patients needing behavioral health interventions and provides these services promptly within the clinic.
  • Develops and implements specific clinical pathways for select populations.
  • Facilitates groups addressing biopsychosocial issues impacting medical conditions.
  • Coordinates care and connects patients with community behavioral health agencies as needed, providing consistent and timely feedback to referring providers.
  • Handles crisis triage and intervention, assisting in de-escalating situations involving patients who are upset or frustrated.

Position Qualifications

  • Current Arizona licensure as a Professional Counselor.
  • Eligibility for credentialing and contracting with all insurers.
  • Prior training in Motivational Interviewing (MI) is preferred.
  • Prior training in Acceptance and Commitment Therapy (ACT) is preferred.
