Licensed Practical Nurses (All shifts)
We are seeking highly skilled and compassionate Licensed Practical Nurses (LPNs) to join our dynamic team at Accela Rehab and Care Center at Somerton in Philadelphia, Pennsylvania. As an LPN at our center, you will play a vital role in providing high-quality patient care and assistance in our rehabilitation and care services.
About the role
As an LPN at Accela Rehab and Care Center at Somerton, you will have the opportunity to work in a fast-paced and dynamic environment with a dedicated team of healthcare professionals. Our center is committed to providing exceptional care and services to our patients and their families. LPNs (all shifts: 7a-3p, 3p-11p, 11p-7a) will work in collaboration with our interdisciplinary team to ensure that our patients receive the highest level of care and support.
Responsibilities
- Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments
- Monitor patient vital signs, administer medications, and document patient care in accordance with established medical records
- Collaborate with the interdisciplinary team to develop and implement patient care plans
- Assist with daily living activities, such as bathing, dressing, and grooming
- Maintain a clean and safe environment for patients and staff
- Participate in quality improvement initiatives to ensure optimal patient outcomes
- Demonstrate respect, empathy, and professionalism while interacting with patients, families, and colleagues
Requirements
- Current LPN license in the state of Pennsylvania
- Effective communication and interpersonal skills
- Ability to work in a fast-paced environment with multiple priorities
- Strong attention to detail and organizational skills
- Ability to lift and move patients as needed
