Licensed Practical Nurse (Weekends)

Salary: $31.25–$34.13 BOE

Schedule: Friday, Saturday, Sunday | 8 AM – 8 PM
Location: Hamilton County, Ohio and surrounding area

Requirements

• Licensed Practical Nursing Degree
• Minimum of 1 year of clinical experience / skilled care
• Hospice experience preferred
• Valid driver’s license and auto insurance coverage
• Reliable transportation; willing to travel

Responsibilities

• Provide skilled nursing care to end-of-life patients during facility and/or home visits
• Deliver on-call services after hours and complete scheduled skilled visits
• Assist registered nurses in carrying out the plan of care
• Perform specialized procedures (as defined in the state nurse practice act) under RN direction and physician’s orders
• Actively participate in interdisciplinary group meetings to keep the patient plan of care current and address identified issues
• Reinforce patient and family education regarding end-of-life care
• Keep the Registered Nurse Case Manager informed of changes noted during visits
• Pursue personal and professional growth through continuing education, workshops, conferences, professional meetings, and individual research
• Perform other duties as directed
