What You'll Do!
- Provide skilled nursing care under the direction of the RN Case Manager and attending physician
- Monitor patient conditions and communicate changes to the care team
- Administer medications and treatments
- Educate patients and families on disease management and preventative care
- Help reduce hospital readmissions through quality patient education
- Collaborate with physicians, case managers, and interdisciplinary team members
- Complete timely documentation in accordance with Medicare and agency guidelines
What We're Looking For
- Current Licensed Practical Nurse (LPN) license
- Minimum of one year of acute care LPN experience (two years preferred)
- Home health experience preferred
- Valid driver's license and active auto insurance
- Strong assessment, communication, and documentation skills
- Compassionate, dependable, and patient-focused
