Licensed Practical Nurse - Sign On Bonus

We are seeking a Licensed Practical Nurse (LPN) in Home Health to deliver quality patient care in patients' homes. Provide clinical services within the scope of practice, following the plan of care, and adhering to infection control procedures.Accurately document patient visits in electronic medical records and visit verification systems.Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.Function effectively in diverse home environments regardless of patient background. Qualifications include: Current, unrestricted LPN licensure in the state of practice.Valid driver’s license, vehicle insurance, and access to dependable transportation. Preferred skills and attributes: CPR certification or ability to obtain within 90 days.Home care experience, strong organizational and communication skills, and ability to work independently.Flexibility to meet client needs and manage multiple tasks.
