About the role

VNS Health Home Care Licensed Practical Nurses (LPNs) deliver personalized, patient-centered care in patients' homes and communities, helping them heal and age comfortably outside hospital settings. Under the supervision of Registered Nurses, they monitor patient progress, assess health changes, and adjust care plans accordingly. Their duties include care coordination, communication with interdisciplinary teams, and providing education and support to patients and families, especially regarding end-of-life care. The role involves community visits, effective time management, and using technology such as tablets and EMRs to maintain accurate records. Qualifications

• Valid LPN license and registration in New York State
• High school diploma or LPN program completion
• Work experience as a new graduate or bilingual skills may be required
