Licensed Practical Nurse, Resident

About the role

Assist in delivering patient-centered, in-home nursing care under RN supervision, focusing on clinical outcomes and personalized support for patients in their communities. Travel to patients' homes or facilities is involved, with an emphasis on effective communication, empathy, and education to empower patients and families, including end-of-life care support.

Responsibilities

• Monitor patient progress and assess needs
• Adjust care plans as required
• Coordinate with interdisciplinary teams
• Utilize technology to maintain accurate records

Requirements

• Valid LPN license and registration in New York State
• High School diploma or LPN program completion
• Work experience as a new graduate or relevant bilingual skills may be preferred
• Driver’s license may be required based on operational needs
