Licensed Practical Nurse/Registered Nurse - Respite

Responsibilities

• Provide education and consultation to clients and staff on medical health, medications, and general medical topics
• Deliver one-on-one training tailored to specific individual needs
• Develop and lead groups and trainings for clients and staff on general health and wellness topics, including nutrition, exercise, and smoking cessation
• Aid clients in accessing community resources that support their health and wellness goals
• Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and available treatment options
• Perform direct medical care for a Person as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc.
• This includes providing interventions specific to Person’s IAP (Individual action plan) for medical/health and wellness goal areas
• Review medical documentation for each assigned client and provide consultation and coordination of care as needed
• Cook up overall medical care for clients and ensure necessary follow-up is completed
• Provide health and wellness interventions focused on education and skill-building
• Support the team and clients by introducing tools for ongoing symptom assessment, evaluating treatment effectiveness, and identifying appropriate responses or next steps

Qualifications

• Bachelor’s degree in Nursing
• Experience working with adults with severe mental illness
• Valid Massachusetts drivers license

Pay

Annual Salary: $60,000 USD - $72,560 USD

Schedule

Monday-Friday, 8am-4pm
