About the role
As the Per Diem Licensed Practical Nurse or Licensed Vocational Nurse in Home Health, you will be responsible for providing quality patient care and performing technical skilled care in the patient's home.
Responsibilities
- Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team (may not alter the plan of care)
- Instructs the patient's family in the care of the patient and maintenance of a healthy environment for the patient
- Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
Required Qualifications
- Current and unrestricted LPN or LVN licensure in state of practice
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
Preferred Qualifications
- Current CPR certification or ability to complete within 90 days of hire
- Home care experience
- Ability to work flexible hours
- Proven ability to work independently
- Proven excellent communication, writing, and organizational skills
State Specific Requirements
- LA: Current LPN Licensure in LA without restrictions
- At a minimum, one year of experience working as an LPN
Pay
- Annual total cash target pay: $43,977 - $65,965 (assumes full-time employment at 40 weekly hours at full productivity)
- Hourly rate: $21.14 - $31.71
- Per visit point rate: $25.37 - $38.06
- Total cash compensation includes earnings from per visit point pay and hourly pay and is based on factors including local labor markets, education, work experience, and may increase over time based on productivity and performance
- This role receives two types of compensation: per visit point rate compensation for visits (calculated by multiplying per visit point rate by productivity points accrued for various visit types, with points inclusive of direct and indirect patient care activities) and hourly rate for certain non-visit activities such as orientation
- Visits are assigned based on patient and business needs; number of visits per week varies based on individual productivity targets and productivity points assigned
