Licensed Practical Nurse - PRN

Responsibilities

• Follows assigned duties provided through the charge nurse on the shift.
• Provides appropriate nursing interventions as indicated for the care and safety of the patients.
• Administers medications according to the prescribed orders in a timely manner.
• Documents the medication and procedures implemented into the medical record following the established protocol.
• Works as a team member to facilitate completion of the admission and discharge process for all patients assigned.
• Transcribes orders accurately and audits the medical record for accuracy of the orders being transcribed and implemented.
• Assumes the role of patient advocate providing education to patients and families as indicated.
• Communicates with other members of the nursing team to fulfill the needs of the patients.
• Assists with activities of daily living.
• Encourages and fosters independence with the patients as indicated.
• Communicates with the medical staff on identified patient issues.
• Performs patient care assessments timely and accurately.
• Addresses patient care needs in a timely manner.
• Documents findings into the medical record accurately and timely.
• Ensures daily documentation is completed thoroughly and signed and dated according to protocol.
• Adheres to Human Resource policies regarding code of conduct and attendance.
• Follows Infection Control and Employee Health Guidelines to ensure safety of patients and staff.
• Actively works with the Service Development Department to facilitate admissions into the facility in a timely manner.
• Functions as a member of the multidisciplinary team and communicates identified patient goals, interventions, and assessments.

Requirements

• Previous experience caring for geriatric patients is essential.
• Med-Surg experience is a plus.
