Licensed Practical Nurse-Primary Care Integrated Clinical Community

About the role

The Licensed Practical Nurse (LPN) provides direct physical and emotional care to Veterans in Primary Care and Integrated Care Community (ICC) at the Saint Cloud VA Medical Center. They perform novice-level nursing activities, support procedures, deliver education, document care, and assist the Registered Nurse (RN) or Provider.

Responsibilities

• Obtain vital signs and complete clinical reminders
• Reconcile medications and perform basic clinical procedures
• Absorb and label specimens, assist with special procedures, and monitor patients during and after procedures
• Document care and observations accurately in the electronic medical record
• Provide education to Veterans and families regarding disease processes, medications, diagnostic procedures, self-management, and health promotion
• Coordinate patient care activities with other departments and facilities
• Collaborate with clinical coordinators, tele-health staff, and administrative personnel to support tele-health programs and health promotion initiatives
• Perform additional assigned duties to ensure safe, efficient, and Veteran-centered care

Requirements

• Graduate of an approved school of practical or vocational nursing
• Active, full, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia

Qualifications

• Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent
• One year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility
• One year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures

Skills

• Prepare, administer, and document actions taken specific to commonly prescribed medications
• Recognize and respond to emergency measures and seek assistance from RN or MD/DO
• Recognize and respond to breakage, malfunction, or loss of equipment, safety hazards, and supply deficiencies
• Recognize and respond to physical and/or emotional changes in patient's condition from prescribed medications/treatments
• Recognize urgent or emergent patient care situations and initiate appropriate emergency interventions
• Recognize and respond to human behavior and patient motivations and reactions to situations
• Communicate effectively with patients, family members, and other staff
• Perform support duties for complex diagnostic tests and/or specialized practices or procedures
• Collaborate with clinical coordinators, tele-health staff, and administrative personnel to support tele-health programs and health promotion initiatives
• Perform additional assigned duties to ensure safe, efficient, and Veteran-centered care
