Licensed Practical Nurse (Poplar)

Responsible for managing the medication system of clients and monitoring their general health.

Responsibilities

• Conduct health screens for all new clients during shelter intake process.
• Use motivational interviewing, recovery, and trauma-informed approaches when delivering services to clients.
• Administer medication and document appropriately in the Medical Administration Record (MAR).
• Ensure documentation in MAR is accurate.
• Notify providers of medication refills needed for clients on a timely basis.
• Inform pharmacy and ensure receipt of prescriptions for medication refills.
• Remove expired or redundant medications from the medication cart.
• Respond to emergencies and ensure follow-up with emergency services.
• Collaborate with the Pediatric Nurse Practitioner (PNP) in coordinating follow-up care.
• Educate clients on medication and treatment compliance strategies.
• Communicate all client medical concerns to the PNP and assist in determining medical care needs and coordination of care.
• Monitor vitals and clients’ response to medications and side effects.
• Review findings with PNP and assist with follow-up medical care as needed.
• Order medical supplies as needed and store supplies and medication records in a secured closet.
• Secure medication cart in a locked, secured location.
• Participate in team meetings, case conferences, staff meetings, and trainings.
• Collaborate with the social service team in assessing clients’ needs, developing service plans, and coordinating care.
• Document delivery of services, client progress, etc., in electronic case records in accordance with agency policies and procedures.
• Perform all other duties and responsibilities as assigned, adapting to changing priorities and newly added responsibilities.

Requirements

• Current New York State Licensed Practical Nurse (LPN) license.
• Current First Aid/CPR certification.
• Minimum two (2) years post-licensure experience providing nursing care in a clinic or social service setting.
• Excellent analytical, evaluative, and human service management skills.
• Ability to remain calm and act effectively in emergency situations.
• Strong attention to detail and problem-solving skills.
• Exceptional organizational and time management skills with the ability to prioritize assignments.
• Excellent written and oral communication, leadership, and supervisory skills.
• Ability to work well under pressure, be flexible, and complete complex projects in a fast-paced environment.
• Capable of developing collaborative working relationships and functioning within an interdisciplinary team.
• Adept and comfortable with crisis management.
• Ability to work effectively with a wide range of community-based providers.
• Familiarity with DHS and OTDA social service documentation policies and procedures.
• Advanced computer proficiency, including Word, Excel, and the ability to learn new technologies and databases.

Other Requirements

• May be required to work long hours and a varied schedule.
• Travel throughout various locations in the NYC area.
• Lifting and moving objects weighing up to 30 pounds several times per week.
