About the role
Provide focused, one-on-one pediatric home health care in patients' homes.
Responsibilities
- Support pediatric patients with skilled nursing needs
- Collaborate closely with RNs and clinical leadership
- Follow detailed care plans
- Ensure safe, consistent care aligned with CDC guidelines
- Deliver personalized nursing care in a home setting
- Work under RN oversight with clear care plans
- Maintain safety protocols and proper use of PPE
- Utilize employee portal to manage schedules and stay connected
- Participate in training, CEUs, and preceptor programs
Requirements
- Valid Pennsylvania LPN license
- TB test
- BLS CPR certification
- Valid driver’s license
- Experience or willingness to train in G-tube, trach, or vent management
