Licensed Practical Nurse Part-Time

Responsibilities

• Maintain and monitor the health conditions of individuals served.
• Ensure quality services are provided by administering medications and documenting accordingly.
• Oversee and monitor staff members who administer medications.
• Provide prescribed medical treatment based on physician’s orders and offer personal care services.
• Conduct assessments and implement health care services as per each Individual Service Plan (ISP).
• Respond to changes in the condition of individuals served and provide necessary support.
• Train staff members on universal precautions, infection control, and medication administration.
• Serve as a liaison and advocate with other medical and professional service providers, coordinating services and ensuring consistent treatment.
• Train individuals served on various topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation.

Qualifications

• A minimum of an Associate’s Degree in Nursing or a related field, or a state certificate.
• One year of experience working with the population served in a nursing capacity.
• Current state LPN or LVN licensure for the state of residence.
• Valid driver’s license, car registration, and auto insurance.
• Ability to perform CPR/FA, HIV/AIDS certification, BBP, and ACT training as required by state licensing regulations.
• Other certifications as required by state licensing regulations.
