About the role
Provide nursing services, monitor health conditions, and ensure quality care for individuals with disabilities or medical needs.
Responsibilities
- Administer medications, document treatments, and oversee staff medication administration.
- Conduct assessments, develop and implement healthcare plans, and respond to changes in individuals' conditions.
- Train staff on infection control, medication procedures, and universal precautions.
- Serve as liaison with medical providers and external agencies to coordinate services.
- Educate individuals on self-medication, treatments, sex education, and smoking cessation.
Requirements
- Associate’s Degree in Nursing or related field, or state certification.
- At least one year of nursing experience with the target population.
- Current LPN or LVN licensure, valid driver’s license, auto insurance, and registration.
- Certifications such as CPR/FA, HIV/AIDS, BBP, and ACT as required by state.
- Effective communication skills with staff, individuals served, and families.
