Licensed Practical Nurse - PACE Care

About the Role

Provide nursing care in a PACE (Program of All-Inclusive Care for the Elderly) Center under RN supervision. Duties include triage, medication administration, lab and procedure assistance, wound care, and patient documentation.

Responsibilities

• Collaborate with the care team to prioritize treatments and provide wound care
• Serve as liaison between providers, participants, families, and staff
• Deliver patient education focusing on pharmaceuticals
• Utilize nursing processes to ensure quality care in accordance with policies and procedures

Requirements

• Valid LPN/LVN license
• Associate degree or vocational graduate
• At least 1 year of experience
• BLS certification within 15 days of hire
• First Aid certification within 30 days of hire
• No travel required
