About the role
A Licensed Practical Nurse (LPN) provides patient-centered care in home settings for Pediatric and Adult patients with G-tube, Trach, and Ventilator needs.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders.
- Monitor signs and symptoms to identify physiologic and psychosocial changes.
- Collect, analyze, and interpret healthcare data, documenting diagnoses and patient status.
Requirements
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
