About the Hospice LPN role
Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients. Provide on-call services after hours, and complete skilled visits.
Responsibilities
- Assist Registered nurses in carrying out the plan of care.
- Assist RN in performing specialized procedures (as defined in the state nurse practice act) as directed by the RN under physician's orders.
- Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
- Assist in the reinforcement of patient and family education regarding end-of-life care.
- Keep the RNCM informed of changes noted during the process.
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
- Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
Requirements
- Licensed Practical Nursing Degree
- A minimum of 1 year of skilled care experience
- Hospice Or Home Health Experience Preferred
- Valid driver's license and Auto insurance Coverage
- Reliable transportation: Willing to travel
Pay & Schedule
- Salary: $31.25-$34.13 BOE
- Schedule: Part-time | Saturdays | 8 PM-8 AM
- Location: Franklin County, Ohio, and surrounding area
