Job Summary

Rosewood Rehabilitation & Nursing Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Weekend Baylor Program team!

Responsibilities

• Provide direct patient care, including administering medications
• Monitor and report changes in patient condition to the registered nurse or physician
• Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
• Maintain accurate and detailed records of patient care and treatments
• Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
• Foster a positive and respectful environment that promotes patient-centered care

Requirements

• Current LPN licensure in the state of Pennsylvania
• Prior experience in a healthcare setting; previous experience in long-term care or rehabilitation setting a plus
• Strong communication and interpersonal skills; ability to work effectively with patients, families, and healthcare team members
