Licensed Practical Nurse (LPN) UP TO $7,500 SIGN ON BONUS

About the role

Golden Hill Nursing and Rehabilitation Center is a 5 Star CMS, 280-bed skilled nursing facility located at 99 Golden Hill Drive, Kingston, NY 12401. We are seeking compassionate, professional, family-oriented Licensed Practical Nurses (LPNs) for full-time and part-time positions on all shifts, including 12-hour shifts (7a-7p, 7p-7a) for days and nights.

Responsibilities

• Admissions, discharge, and transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility policies and procedures, and nursing judgment
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident
• Implements the patient/resident plan of care and evaluates the patient/resident response according to state-specific regulations
• Directs and supervises care given by other nursing personnel in selected situations in accordance with state-specific regulations
• Maintains knowledge of necessary documentation requirements
• Maintains knowledge of equipment set-up, maintenance, and use (e.g., monitors, infusion devices, drain devices)
• Maintains confidentiality and patient/resident rights regarding all patient/resident/personnel information

Requirements

• LPN (Licensed Practical Nurse) licensed by the State
• CPR Certification required
• Two years prior nursing experience preferred
