Licensed Practical Nurse (LPN) Unity Living Center (Full-Time, Day/Evenings)
About the role
Unity Living Center is a 120-bed, state-of-the-art skilled nursing facility located at 89 Genesee Street, Rochester, NY 14611 on two recently renovated floors within the Rochester Regional Health St. Mary’s campus. Our team provides specialized, patient-centered care for individuals with complex medical needs, dementia, and behavioral challenges, creating an environment where nurses can grow their clinical expertise while making a meaningful impact.
As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.
Responsibilities
- Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
- Performs treatments and documents progress of same as per physician
