Licensed Practical Nurse (LPN) Supervisor

Key Responsibilities

• Supervises neighborhoods and manages Home Health Aides in reinforcing elements of residents' plan of care
• Delivers nursing care in accordance with agency policies and procedures
• Participates in incident reporting and follow-up actions
• Assists with resident admissions and conducts daily stand-up meetings with Home Health Aides
• Completes, maintains, and submits accurate clinical records and prepares oral reports
• Absorbs and evaluates nursing care plans
• Observes, documents, and reports changes in residents' conditions
• Performs nursing procedures, including medication administration and treatments
• Manages medication for residents, including changes, new prescriptions, and refills
• Assists Nursing management with the assessments and reassessments of residents to determine health needs

Residents Relations & Support

• Ensures a safe and healthy living environment for all residents
• Undertakes resident/family teaching for self-care and continuity of care

Qualifications/Skills

• Graduate of an accredited practical nursing program
• Valid New York State License as a Licensed Practical Nurse (LPN)
• Minimum of 1-2 years of experience in community nursing, acute care, or adult care facilities
• Supervisory experience and medication management preferred
