Position Details

Location: Brick, New Jersey

Schedule: Full-Time

Starting Pay: $36.10 per hour

Additional Shift Differentials Available: Earn an additional $2.00-$4.00 per hour based on applicable shift assignments

Essential Responsibilities

  • Provide nursing services and clinical oversight within residential group home settings
  • Aid in the development, implementation, and monitoring of individualized healthcare plans
  • Maintain accurate medical records, health assessments, medication administration records, and related documentation
  • Monitor individuals' overall health status and communicate medical concerns, changes in condition, or treatment updates to appropriate team members
  • Collaborate with residential leadership, healthcare providers, and interdisciplinary teams to support ongoing medical needs
  • Assist individuals during physician visits, hospitalizations, specialty appointments, and admissions to rehabilitation or nursing facilities when required
  • Support medication administration processes and ensure adherence to agency policies and healthcare regulations
  • Promote a safe and healthy living environment while advocating for the well-being of individuals receiving services
  • Participate in health-related training, education, and quality assurance initiatives as needed

Requirements

  • Must be at least 18 years of age
  • Graduate of an accredited School of Nursing required
  • Current Licensed Practical Nurse (LPN) license issued by the New Jersey Board of Nursing required and must remain in good standing
  • Minimum of one (1) year of experience as a Licensed Practical Nurse required
  • Valid New Jersey driver's license required, with no more than five (5) points
  • Strong communication, documentation, and organizational skills
