About the role
All Heart Homecare Agency is a 13-year-old home health agency that serves New York families with compassionate and professional home care services. We are currently seeking dedicated Licensed Practical Nurses (LPNs) to provide one-to-one private duty nursing care in patients' homes.
Responsibilities
- Provide skilled nursing care in accordance with physician orders
- Document patient condition and care provided
- Follow patient care plans
- Communicate with the clinical team
Requirements
- Active NYS LPN License
- CPR Certification
- Infection Control Certificate
- Ability to work the assigned schedule
- Home care or PDN experience preferred
- Strong communication skills in English (bilingual in Spanish or Russian is a plus)
