About the Role

Traditional Home Health Care (Pediatric & Adult Skilled) is a leader in the healthcare field, providing exceptional skilled nursing and aide services. We’re seeking a compassionate Licensed Practical Nurse (LPN) to provide high-quality, one-on-one care and support to pediatric and adult patients in the comfort and safety of their own homes.

Pay

$28-$32 per hour

Schedule

• Immediate need in Williamsport, PA: Monday - Friday, 12pm - 3pm for preschool accompaniment
• Immediate need in Jersey Shore, PA: Overnights, 11:00pm - 7:00am
• Morning, evening, or overnight shifts
• 8, 10, or 12-hour shifts (full shift work)
• PRN, part-time, or full-time
• No weekend or minimum shift requirements

Responsibilities

• Provide one-on-one patient care in home or school environments
• Implement and document the plan of care to ensure quality and continuity of care
• Administer prescribed medication, treatments, and therapies
• Perform patient assessments and coordinate care
• Report any changes in client condition to Clinical Manager and physician
• Promote health, teach, and train family members
