Job Description

We are looking for Licensed Practical Nurses (LPNs) to join our team at The Moments of Lakeville. Our facility offers memory care and enhanced assisted living with 24/7 on-site nursing coverage.

Responsibilities

• Ensure the health and safety of all residents.
• Lead and direct your care team, performing necessary care tasks as needed.
• Collaborate with residents, families, physicians, and Moments staff to create a personalized care plan that enhances the quality of life for the resident.
• Show passion for serving seniors and those living with dementia.
• Communicate effectively with residents, family members, physicians, vendors, and RN supervisors.
• Prioritize and organize work effectively and efficiently.
• Commit to a compassionate care philosophy and a whole-person approach.
• Exercise excellent judgment, decision-making abilities, and problem-solving skills.
• Experience with EHR/EMR systems, particularly Point Click Care.
• Support the orientation and education requirements of new staff members.
• Participate in developing person-directed care plans and sensory/cognitive programming within a culture of compassion, respect, and dignity.

Requirements

• Must be licensed with the State of Minnesota as an LPN and maintain current and unencumbered licensure.
• Must complete a criminal background check from the Department of Human Services and not be on the OIG exclusions list.
• Must be screened for tuberculosis.

Other Information

• Position available for Evening (part time and full time) and Weekend shifts.
• Salary range from $30.00 per hour, experience-based, with additional benefits including shift differentials for evening and NOC shifts.
