The Licensed Practical Nurse (LPN) in Home Health provides quality patient care within the scope of practice, performing skilled clinical services in patients' homes. They collaborate with the healthcare team, document visits thoroughly in electronic records, and participate in care coordination by reporting patient needs and changes in condition. The role requires adaptability to diverse home situations and effective communication with patients, families, and providers.

Responsibilities

• Deliver high-quality clinical services following state laws and care plans
• Accurately document patient visits and activities
• Participate in care coordination, case conferences, and communication efforts
• Adapt to various home environments and patient needs

Requirements

• Current, unrestricted LPN license in the practice state
• Valid driver’s license, vehicle insurance, or access to transportation

Skills

• Preferred: CPR certification
• Preferred: home care experience
• Strong organizational and communication skills
• Ability to work independently and manage multiple tasks
