About the role

As a Licensed Practical Nurse (LPN) at Brighton/Suncrest Hospice, you will provide care and services for a group of patients, visiting them where they reside—including private homes, Skilled Nursing Facilities, and Assisted Living Facilities. You will be a contributing member of the interdisciplinary team, offering expertise in psych-social assessments, interventions, and knowledge of community resources.

Responsibilities

  • Deliver high-quality hospice care to patients in their residences.
  • Conduct psych-social assessments and interventions.
  • Collaborate daily with a team of clinicians in a supportive environment.
  • Work independently while managing day-to-day activities, including phone, email, and written communication.
  • Attend meetings with healthcare professionals and ensure compliance with state and federal regulations.
  • Manage time effectively to prioritize visits, documentation, meetings, and urgent patient needs.
  • Learn and utilize an electronic medical records system.
  • Drive to patient locations using reliable transportation (valid driver’s license and auto insurance required).

Requirements

  • Current Licensed Practical Nurse (LPN) licensure in good standing in the state(s) of practice.
  • One year of experience as an LPN, with at least one year in Hospice or Home Health.
  • Ability to work well in a team environment and collaborate with clinicians.
  • Strong time management skills to balance visits, documentation, and urgent patient needs.
  • Willingness to drive and access to reliable transportation, a valid driver’s license, and auto insurance.
  • On-call and triage experience.
