Job Description

The Moments of Lakeville is seeking Licensed Practical Nurses (LPNs) for full-time and part-time positions, including weekend shifts. This is a great opportunity to join a dedicated team providing exceptional care to residents.

About the Role

You will be part of a multi-nursing team ensuring the health and safety of all residents. Your responsibilities include leading and directing your care team, performing necessary resident care, collaborating with residents, families, physicians, and staff to create a person-centered care plan, and supporting staff orientation and education.

Responsibilities

• Ensure the health and safety of all residents
• Lead and direct your care team
• Perform resident care as needed
• Collaborate with residents, families, physicians, and staff to create a person-centered care plan
• Communicate effectively with residents, family members, physicians, all vendors, and RN supervisors
• Prioritize and organize work effectively and efficiently
• Support the orientation and education requirements of staff
• Develop person-directed care and sensory/cognitive programming in a compassionate, respectful, and dignified environment

Requirements

• Licensed with the State of Minnesota as an LPN and must maintain current and unencumbered licensure
• Must complete a criminal background check from the Department of Human Services and may not be on the OIG exclusions list
• Must be screened for TB

Qualifications

• Innate passion for serving seniors and those living with dementia
• Excellent judgment and decision-making abilities and problem-solving skills
• Experience with EHR/EMR (preferably PointClick Care)
• Ability to work with a team to develop person-directed care and sensory/cognitive programming

Pay

From $30.00 per hour with shift differentials for PM and NOC shifts.
