Responsibilities

  • Ensure that each individual receives quality, individualized health care management, resulting in their optimal health.
  • Conduct routine site visits, checking vitals, doing assessments, counseling on medication administration, and documenting progress.
  • Act as a health/medical resource for staff, training staff in the medical arena.
  • Conduct audits and reviews of medications.
  • Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records.

Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of Indiana required.
  • 1-2 years experience working with individuals with disabilities or other long-term care is preferred.
  • Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies.
  • Strong verbal/written communication skills.
  • Ability to work independently and efficiently.
  • Organizational and computer skills.
  • Acceptable background check & driving record check.

Pay

$28/hour

Schedule

  • Full-time, Monday-Friday (40 hours).
  • Typically 8am-5pm, with flexible hours.
  • No weekends.
  • Occasional travel required.
  • Participate in an on-call rotation for triage of medical concerns for 1 week per month.
