About the Role

The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents and supervising the day-to-day activities of nursing assistants. This role ensures the highest degree of quality care is maintained in accordance with federal, state, and local standards, guidelines, and regulations.

Responsibilities

• Direct and supervise the day-to-day functions of nursing assistants.
• Plan and coordinate shift services, programs, and activities with assigned nursing staff and support personnel.
• Make written and oral reports concerning shift activities as required.
• Admit, transfer, and discharge residents as needed.
• Provide leadership to nursing personnel on your unit/shift.
• Complete and file required record-keeping forms/charts for resident admission, transfer, and/or discharge.
• Receive and transcribe telephone orders from physicians.
• Chart nurse’s notes in an informative and descriptive manner reflecting care provided and resident responses.
• Fill out and submit accident/incident reports to the Director as required.
• Perform routine charting duties in accordance with established policies and procedures.
• Prepare and administer medications as ordered by the physician.
• Maintain a good working rapport with inter-departmental personnel to ensure nursing services meet resident needs.
• Create and maintain a warm, calm, and positive environment for residents and staff.
• Receive and give nursing reports at the start and end of shifts.
• Make rounds with physicians as necessary.
• Administer professional services such as:
  • Catheterization
  • Tube feedings
  • Suction
  • Applying and changing dressings/bandages
  • Colostomy and drainage bag care
  • Phlebotomy (taking blood)
  • Massages and range of motion exercises
  • Care for the deceased/dying
• Monitor assigned personnel to ensure compliance with safety regulations for equipment and supplies.
• Maintain confidentiality of all resident care information, including protected health information.
• Report incidents of unauthorized disclosure of resident information.
• Ensure nursing care is provided in privacy and personnel knock before entering resident rooms.
• Perform other duties as assigned.

Requirements

• Licensed Practical Nurse (LPN) with a current unencumbered state license.
• Ability to read, write, speak, and understand English.
• Ability to make independent decisions when circumstances warrant such action.
• Knowledge of nursing and medical practices, procedures, and regulations pertaining to nursing care facilities.
• Current CPR Certification.
• Experience in Long Term Care is preferred.

Physical Demands

• Ability to move intermittently throughout the workday.
• Ability to cope with mental and emotional stress associated with the position.
• Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, personnel, and support agencies.
• Ability to push, pull, move, and/or lift a minimum of 25 pounds to a height of 5 feet and transport such weight a minimum distance of 50 feet.
